Deciding the Titles for the episodes..

Tonight, I was watching the episode "Optimism in the Housing Market" and I noticed that it seems some of the titles for episodes come from a simple line said in the episode (possibly towards the end of the episode) or something a character does. It made me wonder how they come up with the titles when it seems the plot is nowhere near what the title is aside from it being said or an action done by one of the main characters. Episodes I noticed this:

Who's That Dancing with Nora Batty (though not a phrase, but a scene at the end of the ep)
Whoops
Why Does Norman Clegg Buy Ladies' Elastic Stockings
When You Take a Good Bite Yorkshire Tastes Terrible
Phantom of the Graveyard (scene at the end)
Nowhere in Particular
How Errol Flynn Discovered The Secret Scar of Nora Batty (kinda)
Optimism in the Housing Market
 
One of the strangest titles for me was 'Who's that with Barry and Glenda,It's not Barry and Glenda'
 
Quite a few titles are slightly altered from well known books or plays or films.

A Certain Smile for example
 
another title changed slighty was "Three Men and a Mangle" when "Three Men and a Baby" was in cinemas at the same time
